This form allows a designated adult to give consent for emergency medical treatment for a child if neither parent can be contacted. Many parents give completed Medical Consent Authorization forms to daycare personnel, babysitters or temporary guardians. PAN's Medical Consent Authorization form includes the requirements of state law [11 Pa.C.S. §2513] and provides space for acknowledgment by the maker and affidavits of two witnesses.
