When you need the latest version of a motor vehicle or driver licensing form, go to www.dmv.pa.gov and click the "Forms and Publications" tab in the upper right corner of the page.

You can filter the list of items by choosing a Document Type or a Category from the drop-down lists and clicking the Filter button. For example, choosing the Document Type "Fact Sheet" and the Category "Insurance" displays only fact sheets that pertain to insurance. To search again, click the Reset button. You can also sort the list by Document Number, Title, Document Type or Topic by clicking on those column headings.

Controlled multi-part forms are not available online. These forms can be completed only by an agent, dealer or messenger who is authorized under contract with PennDOT. Other than controlled forms, you can always get the latest version of a PennDOT form, fact sheet or publicaton on the Web site.
