Look Out for Wisconsin Form MV2115 with Title Applications

Wisconsin discovered motor vehicle wholesalers are using Wisconsin Form MV2115 to reassign titles and these forms are being accepted by other states.

Wisconsin Form MV2115 is only allowed to be used when accompanied by one of the following:
  • Wisconsin title issued before 5/1/90;
  • A non-conforming title issued by any other state; or
  • A non-conforming Manufacturer Statement of Origin.
Wisconsin is asking other jurisdictions to not accept this form unless it complies with one of the scenarios above.

If agents see this form with applications for Pennsylvania Certificate of Title, the agent should reject the application if it does not comply with one of the acceptable scenarios.
